CLOSED FOR TRAINING EVERY 2 MONTHS

TARGET is a scheme that brings our city’s GPs, Practice Nurses and Health Care Assistants together every two months to learn new skills, hear about new treatments and share good practice.  We think it will mean better services all round – that is what we’re aiming for!
It means that your surgery will need to close for one afternoon every two months but we will ensure that medical care will be available when you need it.  The surgery will therefore be closed on the following afternoons but the Out of Hours service (which can be accessed by calling 111) will cover:
Wednesday 13 November
Thursday 16 January 2020
Thursday 26 March 2020

Travel Vaccinations - 8 Weeks Notice Required.  Charges are Incurred.  global air travel

If you require any vaccinations relating to foreign travel you need to complete the form below and hand this/post to the practice.  The Practice Nurse will then review your travel information and check your medical records with regards to your travel plans.  The Nurse will then call you to  discuss what vaccinations (if any) are required and book you your appointment to come in for the vaccinations to be administered.  We require an individual travel questionnaire for each registered patient travelling.

Travel Questionnaire - 1 Form to be completed per Registered Patient Travelling

There is further information about countries and vaccinations required on the links below

Europe Europe & Russia North America North America
Central America Central America South America South America
Caribbean Caribbean Africa Africa
Middle East Middle East Central Asia Central Asia
East Asia East Asia Australasia Australasia and Pacific

It is important to ensure that the travel questionnaires are completed as early as possible.  Travel vaccines have to be ordered because they are not a stock vaccine. .

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
